How to design a survey form with a likert appearance?

You could use the likert from the appearance to display your response options as outlined below:

If you are using the form builder UI:

• Design a normal select_one question type as shown in the image below:

• Select likert from the Appearance (Advanced) as shown in the image below:

If you are using the xlsform:

• In the survey tab of your xlsform:

• In the choices tab of your xlsform:
