Seems a known issue in Enketo, see Modify font color and family - #11 by Josh.
Some further hints for formatting can be found here Enketo does not accept text formatting scripts as KoBoCollect android app - #4 by wroos.

As you can see in the link, it does NOT work in Enketo so far for choice-items.

I resolved this issue by inserting the options on the first question only, it flowed to all parts of the form.
