I am unable to download xls form my survey. Tried the icon. Anyone can help?

Dear kobo team, I was able to download xls form but I can’t access my questionnaire on my android phone. Below I attached screenshot.
@edework, did you mean you are not able to get the blank forms to your Collect Android App? Did I get you correctly?
Yes, there are 2 forms I created. As you can see, when I request a blank form, only one is available.
@edework, could you provide me the following through a private message? Maybe I could have a closer look at your case.
- Username
- Project name
- Server

@edework, when reviewing your XLSForm I could see a syntax issue under the choices sheet. FYR …
A label missing. Fix this and then validate it against this online validator to see if there are any other syntax issues. The online validators should help you identify syntax issues if any within your XLSForm.
