There should be a choices sheet in this xlsform. When looking for a sheet named ‘choices’, the following sheets with similar names were found: 'choices '. Please ensure that the choices sheet has the mandatory columns ‘list_name’, ‘name’, and ‘label’

Welcome to the community, @emykobo! Have you validated your XLSForm through this online validator? The online validator should help you identify syntax issues, if any, that are affecting the form deployment.

This post discussed previously should help you use the online validator.

Maybe share a screenshot of the error message you see when validating your XLSForm through the online validator. The community should then be able to help you out.

Hi @emykobo,

I think you imported your xls file to Kobo and trying to deploy it. Please check this excel file, there might be two or more choices sheet that is causing the problem. Or the choices sheets doesn’t have the ‘list_name’, ‘name’, and ‘label’ columns. You also can the validator as suggest by the Kal_Lam
