I tried to upload the attached xlsx file to this survey form. But I got the message: “Index Error: pop from empty list.” May I know why this is the case and what I should do to solve it?
I would also like to upload this Excel to this post but KoboToolbox Community stated that I cannot upload attachment. Please let me know how I can upload it too.

Hi! The error “Index Error: pop from empty list” usually means there’s a formatting issue in your XLSForm — often something like a missing value in the type, name, or label columns, or an extra blank row that Kobo tries to process as a question. I recommend checking for:
- Blank rows in the survey or choices sheets
- Missing or duplicate values in the name column
- Incorrect or incomplete question types
You can also validate your form using ODK’s XLSForm Online Validator to pinpoint the exact issue.
As for uploading the Excel file here, Kobo Toolbox forum doesn’t allow .xlsx attachments, but you can upload your file to a cloud service like Google Drive or Dropbox and share the view-only link here so others can help review it.
