Welcome back to the community, @dwangola! Have you validated your XLSForm through this online validator? The online validator should help you identify syntax issues if any that are affecting the form deployment.

@okware_256, all you need to do is follow the steps outlined below: Open online validator. This screen appears as shared below: [Image 1] There press Choose File and upload your XLSForm. Once the XLSForm is uploaded there, press Submit. As soon as you press Submit, you will see something as shown below: [Image 2] Now press Preview in browser. @Kal_Lam thanks for the this. Since this was a redeployment of v2 we were a bit stumped, but after using the online validator we were able to identify other errors. One thing we also noticed was that kobotoolbox was changing the names of the field in the form that was tested in ODK without any issues in ODK. This time we used Kobotoolbox and adjusted all the names to avoid irregularities.
