thank u Kal_Lam, I have developed one from scratch. can I save that and re-use it again for different survey with different name or can I copy paste when needed? if so, how? thank you

Maybe the best option is to download your survey form as xlsform and reuse it whenever needed. You could download your xlsform as discussed in our previous post:
