Matrix question does not display properly in PC browser and Iphone safari

Dear Kobotoolbox teams

I try to build a Matrix question, but it looks strange when open in PC web browser and Iphone safari.

I also used Kobo 2 years ago and could easily built a matrix as below.

I have not used Koto since then, I have a feeling that some question types are no longer available…

Welcome back to the community, @buithanh! Kindly please be informed that data collection for matrix questions is best suited for a device that has large screens when collecting data. You should be able to learn more about the same through this Enketo blog here.
