Welcome to the community, @kenmugy! This looks a bit different from what you’re referring to. However, if you’d like to design a question that allows selecting two or more choices, please try using the select_multiple question type.

the image i pasted above is from when your creating a new project when selecting multiple countries (see image below)
would it be possible in the form to have them appear as the image as i initially shared
