Push Rapidpro information to Kobo

Dear Kobo users,

I am facing an issue with Kobo, i would like to automatically push SMS survey response to kobo automatically. In RapidPro, pushing data is possible using webhook/Post functionality; my main issue is to structure the url of the form/submission.

can somebody assist me?
