Ranking and Matrix

My survey needs top 5 answers. From the top answers, respondents need to select choices:

Q1: What are the top 5 reasons for dropping out?
Choice 1 _______ === respondent selects 1 from the 15 selections
Choice 2 _______ === respondent selects 1 from the 15 selections
Choice 3 _______ === respondent selects 1 from the 15 selections
Choice 4 _______ === respondent selects 1 from the 15 selections
Choice 5 _______ === respondent selects 1 from the 15 selections

Once the 5 answers are selected, the choices should appear like this in the succeeding question (Q2):

How to do this?

So you mean, you have the following:

  • Q1, is a select multiple question where you are allowed to select any 5 choices from a list of 15 choices that is available.
  • Q2 should be in a matrix form where you are able to rank the first choice in the first row, similarly, the second choice in the second row, third choice in the third row, fourth choice in the fourth row, and fifth choice in the fifth row.
  • And you also should be able to see the PRE-PANDEMIC and DURING-PANDEMIC column in the matrix.
