Regarding the follow up for the same participant's entry

I want to know what is the procedure to create follow-ups.
like in a form created if I do an entry for a person and then after 10 days I have to again go back to the same entry (ID) and enter new data for the same person as his/her follow-up record. How to do that?

Yes Sir I want to do the follow-ups of the same participants. At least 2-3 follow ups.
