Removing borders and aligning

Hi community, May I ask for your help? I have to make an eq5d vas scale. it should look like this:

but so far I was only able to make it look like that:

Is it possible to remove borders from the grid and change the location of the text (like center or align left)?
Can I change the way how the number shows on range scale (I have to do something like this black box with your health number showing up
Maybe you have better idea how it may be done?

@Malgorzata, you may not be able to control the form on how it looks as most of the control is made by the system. You should however, be able to display it vertically, as shown in a post shared below:
