Search function with select_one_from_file question type error

hello , this is my second post about the subject apologies
i have been trying to implement the search function from an external CSV into a select_one_from_file but it doesn;t work and i cant seem to find the problem
the error is no such column a_50(code 1)

here is the tool and the CSV file a help would be appreciated it is getting frustrating !
