I already tried validating the form using the online validator and fix some errors but the server problem still prompted.
I also tried re-building a previously deployed form to check if its really a form design issue but the same error was encountered. Please help. Thanks!
I suspect you may simply be hitting the limits of some browsers with this provided form. Specifically, there are 5 different language permutations, and one of your choice lists - barangay - has over 42,000 entries (!).
When I pruned out the additional languages, and significantly reduced the number of options in this one internal choice list, I was able to successfully deploy your form for Kobo [although it is missing a number of additional external required cvs files, so I could only preview it…].
I’m not suggesting you be as brutal as me and prune everything out as I have done - I simply wanted to confirm there were not any actual intrinsic errors in your form; and there appear to be none. But you may still need to refactor this form somewhat to make it actually deployable, in terms of its huge size.

I’ve followed your advise and tried the same but the form I’ve got where this issues keeps coming up and it’s not deployable is quite small with only 2 languages. Is there a chance you could take a look? I’m getting no-where with it and it’s blocking some time sensitive activities, so I’m fairly stuck. thank you
Unfortunately, although what you are attempting might seem to be obvious, you cannot in fact provide translations for the form title itself (!) [although this is actually something we are looking at right now, since it is not an infrequent requested feature!]
If you remove this from your setting sheet you should be able to deploy your form successfully. I do note, however, that your translation column headings aren’t quite correct - in particular you should include the short-hand language code as well (eg ‘(en')’, ‘(fr)’, … although this doesn’t appear to throw any errors if you dont). Once I made these changes (below) your form deployed, ran, and submitted fine for me:
I only encounter that problem when I upload and deploy the form using professional plan account. But when I tried to deploy the same form in my free account, it was successful.
