I am trying to create a question in a table format that would have a name, time called and time arrived - is it possible to have all of that information in one question? This is the question below that I would like to copy into Kobo

Hi Casey,
In your form these all need to be separate questions, so in your example there would need to be 12 questions total. The question is how you design it to make your form easy to use. I would suggest putting the three questions in each row inside a group and display it on the same question, naming the group based on what’s written in the first column (Attending MD, Nurse 1, etc.).
