Does it work in Collect if you use either autocomplete (or search) or choice-filter?
You may try using search appearance.
Or just work with a simple cascading select?(See XLSForm Template - Few missing Appearances - #2 by LN - Support - ODK Forum and ODK Template:
ODK XLSForm Template - Releases - ODK Forum).
In your (autocomplete) approach, the user still has to type and know the facility, in correct spelling.

There might also be a filter value missing in row 849 in your choices sheet.
