@sibusiso, when having a closer look at your xlsform, I see that your choices tab has labels missing:
Could you check them out and either add the labels if needed or delete the list_name if not needed? You will need to repeatedly validate your xlsform through this online validator after fixing the syntax issues until you see no more syntax issues.
