@wirdawati, I observed two issues in your shared documents. Fix them and it should solve your issue.
Issue 1: calculation header missing in survey tab of your xlsform
Solution 1: placing calculation header which was missing in survey tab of your xlsform
Issue 2: CSV format not supported by the system
Solution 2: making a CSV format that is supported by the system
