@hamuza_kavuma, seems like your XLSForm that you are trying to upload is not in the XLSForm standard. The XLSForm should have at least a survey sheet in it with choices and settings sheets as optional, based on the questions you have under the survey sheet. You should be able to learn more about XLSForm here. BTW, you can also always validate your XLSForm through this online validator to see if your XLSForm has any syntax issues in it.
