Do you use XLSForms or the web-based form builder to develop your forms? Either way, when you have select-one or select-multiple questions, the key is for each item in your choice list to have a different “name” (i.e. the name used under-the-hood, which is different from the label that is shown to the enumerator).
If you are able to share your XLSForm to this forum then community members may be able to help troubleshoot.

Thank you for sending the Excel so promptly!
I’ve had a look at the “choices” sheet and highlighted the two places where there are duplicates:
These choice lists relate to questions 2.12 and 2.16 - if you go into the answer lists for these two questions and remove the duplicates then you shouldn’t receive this error any more.
For 2.12 you just need to delete the second “Rice” answer.
For 2.16 you will need to edit the “name” fields to be e.g. household_spends_less_on_staples and household_spends_less_on_medical - as currently both are household_spends_less_money_on so both answers have duplicate “names”.
