Hi there, ok in what you shared, i would have to develop the form in excel and upload it? So here is what i have developed in the form builder (not excel). For each of the categories selected e.g. for AGE, i want to assign a score. The same would apply for Height, RCD, Colour, plug integrity, and disease. I then want to sum these scores up. Please could you assist?
