Hiding options from a select one question that has already been answered in the previous question

It is in my shared xls form shared in my post above. If it can not be done what I wanted, it is fine. Still, I can resist selecting one option more than once.
Here, I have come up with another way to do it. I am half done. I was able to eliminate the selected options from first question. I just could not make the same for second question.

Brief overview:
In Q-1, four options are: A, B, C, D. When ‘A’ is selected in Q-1, only options ‘B’, ‘C’, and ‘D’ appears in Q-2. This is what I was able to do. But, to hide another options out of these remaining three for Q-3, that’s what I could not make it work. When ‘B’ is selected in Q-2, then only ‘C’ and ‘D’ should appear as options in Q-3. Would you mind have a look for just one minute? Question is attached here for better understanding. Thanks in advance.
