I want to ask you how to create a question that have 3 answers already and all answers must be give the score from 1 to 5. I have created it in a group or created with Note but it is difficult to show data in Table, it is not showing the Group or Note when we collect and analyze data.
Example below:
During the period from 2017 to 2025, new cases of HIV infection did not decrease as expected. There are 5 reasons that contribute to the new cases of HIV infection did not decrease as expected. Please give the Score in all answers. (Smallest score is 1 and the biggest score is 5).

In the question B put Score from 1-5
